Lisbon - Attorney John T. (Jack) DeFazio, 75, passed away at 5:35 AM Thursday, February 18 at Salem Regional Medical Center following a lengthy illness.
Mr. DeFazio was born June 20, 1945 on Staten Island, NY, a son of the late Carmine and Rosa (LePro) DeFazio.
A 1962 graduate of Augustinian Academy on Staten Island, he earned a B.A. degree in history from Fordham University in 1966. He earned his law degree in 1969 from the University of Virginia then served until 1973 in the U.S. Army Judge Advocate General’s Corps.
He joined the law firm of Green, Schiaivoni, Murphy & Haines in 1973, and in 1980 opened his own practice in Lisbon. In 1992, he joined the Trial Division of Nationwide Insurance where he worked until his retirement in 2010.
A Catholic by faith, Mr. DeFazio was a member of the Salem First United Methodist Church.
He was an avid New York Yankees fan and because of his love for baseball, played many years on the senior men’s league Salem Legends (formerly known as Salem Old-Timers).
Mr. DeFazio is survived by his wife, Catherine (Cathie) Shoop DeFazio, whom he married January 15, 1994; his daughter from a previous marriage, Diane DeFazio of Salem, and several nieces and nephews.
He was preceded in death by a brother, Ernest DeFazio, of Staten Island, NY.
Due to Covid-19, a memorial service will be held at a later date at the Salem First United Methodist Church.
In lieu of flowers, it was his wish that any monetary donations be made to the Second Harvest Food Bank, 2805 Salt Springs Road, Youngstown 44509 or the general fund of Salem First United Methodist Church, 244 S. Broadway (Box 463), Salem, Ohio 44460.
Arrangements are in the care of the Weber Funeral Home, Lisbon.
